Preserve and Revitalize Your Natural Tooth
When the inside of a tooth is damaged, whether from decay, injury, or infection, it doesn’t always need a root canal or extraction. Regenerative endodontics and vital pulp therapy are advanced, evidence-based treatments that help preserve or restore the health of your tooth’s pulp.
Using the latest techniques and materials, we can stimulate healing, reduce the need for invasive treatment, and support long-term tooth development, especially in younger patients.
Regenerative Endodontic Therapy
For immature teeth with necrotic (non-vital) pulp, regenerative endodontic therapy focuses on restoring living tissue rather than removing it. After disinfecting the canal with minimal instrumentation, we induce controlled bleeding to create a scaffold for stem cell activity. This process promotes the regeneration of pulp-like tissue and allows the root to continue developing — strengthening the tooth over time.
Vital Pulp Therapy
When the pulp inside your tooth is inflamed but still alive, vital pulp therapy (VPT) offers a conservative alternative to root canal treatment. We remove the damaged or decayed portion of the tooth, place a biocompatible material to protect the healthy pulp, and seal it to encourage healing. This approach helps preserve the tooth’s natural vitality, stimulates dentin repair, and maintains normal function.
Cape Fear Endodontics Reviews
You know how a flight attendant will stand in front of everyone on the plane and explain safety procedures and other information. This team was just like that with every step and movement. I was informed of everything before they did it and they were very professional. Bravo!!!
Very personable and thorough about the procedure I was having done. Made me very comfortable, and made it easy for me to relax without any medications. Better experience than I imagined.
Dr. Mihle is a very courteous and knowledgeable endodontist. His staff is also top notch. I would recommend his services to all patients needing his speciality.
Professional and transparent in all respects. My first root canal was easier than the anxiety I was feeling.
Everybody was professional and attentive to details. Dr Mihle walked me through the entire process and gave me my options. This was probably my third or fourth visit, and I've been satisfied each visit. I wouldn't hesitate to recommend your office.

Frequently Asked Questions
Where does the biocompatible material used during VPT come from?
The materials used in vital pulp therapy are specially designed medical-grade compounds created to be safe and compatible with the body. They are not taken from another person or donor source, but instead are carefully engineered to support healing and protect the tooth. Our team selects materials that are well studied and known for promoting a healthy response within the tooth.
Does VPT feel the same as a root canal?
From a patient perspective, the experience can feel similar in that the area is numbed and the goal is to keep you comfortable throughout. However, VPT is typically less extensive because it focuses on preserving as much healthy pulp as possible rather than removing it entirely. Our team will walk you through what to expect so you feel prepared and at ease during treatment.
How soon will I know if my regenerative endodontic treatment was successful?
Success is usually evaluated over time rather than immediately. Follow-up visits and imaging allow us to see how the tooth is healing and whether normal function is being restored. In the weeks and months after treatment, our team will monitor changes and look for signs that the tooth is responding well.
