Appointments can be made through our website or by phone. If you are unable to keep an appointment, please give us at least 24 hours' notice so we can offer that time to another patient. If you are a new patient, please download and complete the medical history form and privacy policy, and bring them with you to your first visit.
Although we only participate with Aetna, we will gladly file your insurance information for reimbursement to our office and to you. The total amount paid for procedural costs will vary depending upon your dental plan, but our staff will gladly contact your provider, research your dental coverage and verify the out-of-pocket expenses. Please keep in mind there is no guarantee of payment from your insurance company until the claim has been received by your provider. Please be sure to bring your dental insurance card with you to all visits.
